Welcome to the Center for Ears, Nose, Throat & Allergy.  We are a group of dedicated and compassionate specialists and subspecialist who look forward to meeting you.

Please note which office you have been scheduled for.  Also allow yourself extra time if you are a first time visitor to our offices since traffic and finding your way may take a bit longer than you anticipate.  There are a several forms that you are required to complete before seeing the doctor.  These can take 5 to 30 minutes to complete depending on the complexity of your medical problems.  If you like you may download these forms below and bring the completed forms with you to save time.

Bring all your medications or an updated complete list of all your medications including over the counter medications and a list of your drug allergies. This initial office visit is a consultation and a letter summarizing your visit will be sent to your physician.  However you do not need a referral from your doctor unless your insurance company requires it.  Other than a biopsy, minor or major surgery is not done on this initial visit unless it is a emergency.  The date of a procedure will be arranged by the nurse either during the visit or by phone after the appointment.

Insurance coverage and requirements can be confusing and frequently change.  Our scheduler can identify whether we are in network for your insurance, but it is your responsibility to ensure that you are in compliance with the insurance’s requirements and it is best to check your policy manual or call your insurance representative to determine your coverage and financial responsibilities.  Many times there is a co-pay that you must pay at the completion of the visit. 

We are a busy office since we strive to get patients in on a timely basis.  If a conflict arises please call to reschedule at least a full weekday ahead to allow us to offer that appointment time to someone else.  There is a penalty fee if you fail to show for your appointment.  It is essential that you arrive early enough and have everything completed by your appointment time so that the doctor can be timely.  If you are late you may be asked to reschedule later in the day or on a different day to avoid delaying other patients.
