North Meridian office

North Meridian is US 31 North, a very busy 4 lane highway. 
When approaching from the North, turn right on Carmel Drive, formerly 126th St.  Proceed through the turnabout 270 degrees and go south on Illinois St.  On the left hand side there are 2 identical 3 story office buildings.

Turn left into the parking lot and take the elevator to the 3rd floor.
If coming from the South, you will turn left on Carmel Drive, formerly 126th St.  Proceed through the turnabout 270 degrees and go south on Illinois St.  On the left hand side there are 2 identical 3 story office buildings.  Ours is the second.  Turn left into the parking lot and take the elevator to the 3rd floor.
Alternatively it may be better when coming from the south to turn left at 116th St. and then 90 degree turn through the roundabout to go north on Illinois St.  Do not turn into the hospital.  Go about  ½ mile to the 2 identical 3 story office buildings.  Ours is the first.  Turn right into the parking lot and take the elevator to the 3rd floor.

We are in the professional building attached to Clarian West Hospital
When coming from Indianapolis take Exit 13A onto 10th St. heading west.  After several miles it becomes a 2 lane road and residential and eventually rural.  Just about when you think you gone too far you will see the large hospital on the right.  Turn into the main entrance and proceed past the hospital to the professional building.  We are on the 2nd floor.
Avon Office

Visitors from the west may approach via Interstate 74 or 70 or Rockville Rd (U.S. 40) and take S.R. 267 to 10th St (East 100N) and proceed east to Ronald Reagan Parkway.  The large hospital is on the left.  Turn into the main entrance and proceed past the hospital to the professional building.  We are on the 2nd floor.

 

 

 

 



7440 North Shadeland Ave., Indianapolis, Indiana 46240

This was the previous location of our main office.  This limited office is utilized by Dr. Borrowdale every other Wednesday and is in Drs. Hicks and Ostrowski’s office.
This office is best approached by interstate 465 or 69.  If coming from the north and heading east on 465, exit south on Binford Blvd and immediately get into the left turn lane for the first stop light and turn on 75th St.  The next stop light turn right and turn in the first drive on the right (the same drive for Starbucks). 
Shadeland OfficeThose coming south on 60 will not exit but continue on Binford Blvd.  Immediately get into the left turn lane for the first stop light and turn on 75th St.  The next stop light turn right and turn in the first drive on the right (the same drive for Starbucks). 
If you are coming from the south on 465, take the 56th St. /Shadeland exit.  Carefully follow the signs and go north on Shadeland Ave.  Just before the stoplight on 75th St., turn left into the drive shared with Starbucks.
